Not Feeling Like Yourself and No One Knows Why?
Feeling tired, foggy, or just off for months is frustrating, and it gets worse when a quick visit ends with 'your labs look fine.' You know something is different, and you deserve someone who keeps looking.
Our nurse practitioners take the time to hear the whole story, then order the lab work that fits it. Evans patients leave with a clear next step and a provider who follows up until the answer makes sense.

Spend Real Time With Your Provider
Evans’ Best Primary Care Office
We are here for you
Welcome to NOCO Healthcare. We were founded because we believe patients should be treated as a person, not a unit. What ever you need we are here to help and if we can’t we will make sure we find who can.
Sheree spent 25 years in healthcare and experienced the shift from care initiatives to unit initiatives. She felt she could give much more back to her community and therefore she took the risk and started NOCO Healthcare.
Call our office and you will experience what so many Northern Coloradans have experienced. We will put you first and make sure you are heard and cared for.
Evans patients can also book a virtual visit when the drive is the hard part, and families can read our pediatric care for Evans page for how we care for children.
Remember the days when your provider felt like a trusted friend? We do, and that is what we are bringing back to Northern Colorado.
Here’s a few interesting facts about Evans:
- Evans has over 300 days of sunshine each year and a full four seasons… But Shhh
- Residents enjoy over 300 acres of parks and open space
- Evans has some of the best schools in Northern Colorado
- Back in the day, Evans was the only town in Colorado that would allow liquor and gambling
We love serving our Evans community and would love to be your chosen primary care office.
Blood Pressure Creeping Up Every Year?
A reading that inches higher at every visit is easy to brush off until it isn't. Many adults quietly worry about it because nobody has ever walked them through a real plan.
We track your numbers over time, adjust medications thoughtfully, and talk through everyday changes that matter, like using those 300 acres of Evans parks for a daily walk. The goal is steady numbers and fewer surprises.

Health risks that come with living in Evans
Evans sits just south of Greeley along the South Platte River at about 4,630 feet, close enough that NOCO Healthcare is the primary care office for a lot of families in Ashcroft Heights, Grapevine Hollow, Tuscany, Riverside Meadows, and the older blocks around City Park and Riverside Park. A few local realities shape the care. Flu came early this year: Colorado reported 58 flu hospitalizations through the week ending October 10, 2026, ahead of COVID-19 (39) and RSV (4), and the state confirmed its first pediatric flu death of the season on October 15. The 2025-26 season was Colorado’s worst on record, with nearly 11,700 respiratory hospitalizations.
West Nile virus is the summer risk on the river side of town. Weld County’s mosquito monitoring Zone 1 covers Evans, Greeley, Kersey, and LaSalle, and mosquitoes in all three county zones tested positive in 2026; the county confirmed its first 2026 human case at the end of August after investigating 34 cases in 2025 and 52 in 2023. Radon is the quiet one: about half of Colorado homes test above the EPA action level, and Weld County gives out free test kits each January at weld.gov/go/radon or 970-400-2226.
Evans does not have a hospital. The nearest emergency rooms are North Colorado Medical Center at 1801 16th Street in Greeley and UCHealth Greeley Hospital at 6767 W. 29th Street, both about 10 minutes from the Evans Community Complex at 1100 37th Street. For everything short of an emergency, NOCO Healthcare in Greeley is a short drive up 23rd or 35th Avenue, sees most sick visits the same week, draws labs in the office, and handles follow-ups by video.
Prescriptions go to the pharmacy Evans patients already use: the Walmart pharmacy at 3103 S. 23rd Avenue on the Evans line or the Sam’s Club pharmacy at 3247 23rd Avenue. NOCO Healthcare accepts Health First Colorado (Medicaid), Medicare, and most major plans, and the Spanish-speaking front desk and provider team matter in Evans, where a large share of families speak Spanish at home.
