Same nurse practitioner you would see in the office. Different room.

Virtual primary care for Severance, from a family medicine practice

Severance is growing faster than its healthcare options, and new families often find themselves driving to Windsor, Fort Collins, or Greeley for everything. A virtual doctor visit with NOCO Healthcare takes the drive out of the routine stuff. We are a family medicine practice about 20 minutes away, and our nurse practitioner handles colds, infections, refills, and follow-up questions by secure video, usually the same day you call.

Same-day and next-day video visits are available Monday through Friday. We text you the link.

Key points

  • Same-day and next-day video visits for Severance patients, Monday through Friday, 8 a.m. to 6 p.m.
  • If you need to be seen, our Greeley clinic is about 20 minutes away and we hold appointments for that.
  • Prescriptions are sent electronically to the pharmacy you choose.
  • Medicaid, Medicare, and most major insurance accepted. Self-pay rates available by phone.
  • No app. We text you a secure, HIPAA-compliant link and it opens in your browser.
  • Visits are with a nurse practitioner from our family medicine practice, not a rotating telehealth pool.

Conditions we treat by video

A good rule of thumb: if you would describe the problem to a nurse over the phone and expect useful advice, it is probably a good fit for a virtual visit. For Severance patients that usually means respiratory infections, sinus pressure and congestion, sore throats, earaches, pink eye, urinary symptoms, migraines, rashes and skin irritation, allergies, and stomach bugs. It also covers the unglamorous but important stuff: refills on a blood pressure or thyroid medication, a follow-up on lab results, or a check-in on a condition we are already managing together.

Because we are a family medicine practice rather than a telehealth app, the visit connects to the rest of your care. If the video visit turns into a lab order, an X-ray order, or an in-person appointment, that happens inside the same chart with the same provider. Nothing gets lost between a stranger on an app and your actual primary care.

What does not belong on a video call: chest pain, shortness of breath, injuries that might involve a fracture, wounds that may need stitches, severe pain of any kind, or anything that feels like an emergency. For those, call us or go to the nearest emergency department. We will always tell you when a screen is the wrong tool.

What to expect on the call

Scheduling starts with a phone call to (970) 888-0399. We ask a few questions, find a time, and text you a secure link before the appointment. There is nothing to install and no account to set up; the visit opens in your browser on a HIPAA-compliant platform. If you have never done a video visit, plan on it taking about as long as a normal office visit without the waiting room.

Our nurse practitioner runs the visit the way she would in the exam room: what is going on, how long, what you have tried, what else is happening with your health. Good light helps if you need to show a rash or a throat. Prescriptions are sent electronically to the pharmacy you name. Lab or imaging orders go into your chart. If it turns out you need to be seen, we schedule that before you hang up, and the drive from Severance to Greeley is about 20 minutes.

The worst part of being sick is usually the logistics.

Finding a ride, finding childcare, finding an hour you do not have. A virtual visit removes all of that. You need a phone, a quiet corner, and ten minutes.

A note from Sheree

I know how much it takes for a parent or a working person to get to an appointment. If I can save you that drive on the days it is not medically necessary, I will. And on the days it is necessary, I will say so plainly. That is the deal I make with every patient.

When we will ask you to come in

Virtual care is a tool, not a replacement for primary care. Annual physicals, sports physicals, immunizations, DOT physicals, injuries, minor procedures, and anything that needs listening to your lungs or feeling your abdomen still happens in person at our Greeley clinic. Our primary care for Severance page covers what those visits look like. The drive is about 20 minutes, and we keep same-day and next-day appointments open for people who need them.

The nice part of using one practice for both is that nothing has to be repeated. The provider on your video call is the same one who will see you in the office, with the same chart in front of her.

Insurance and cost

We accept most major insurance plans, Medicare, and Medicaid, and virtual visits are billed the same way an office visit is. Many plans now cover telehealth at the same rate as in-person care; if yours is different, we will tell you what to expect before the visit rather than after. If you do not have insurance, ask about self-pay pricing when you call. We would rather quote you a number than have you skip care over a question mark.

Why new Severance families start with a video visit

Finding a primary care provider in a new town is a chore, and a lot of Severance residents put it off until someone gets sick. A video visit is a low-effort way to meet us and get the immediate problem handled. From there, our Severance primary care and family medicine pages explain how we care for the whole household, and our women’s clinic page for Severance covers women’s health.

Virtual visit questions from Severance patients

How do I book a virtual doctor visit from Severance?

Call NOCO Healthcare at (970) 888-0399 during office hours, Monday through Friday. Tell us you would like a video visit and we will find you a time, often the same day. We text you a secure link shortly before the appointment.

Can my child have a virtual visit?

Yes. Pediatric video visits are common and useful for fevers, rashes, coughs, and pink eye. A parent holds the phone, shows us what is going on, and we decide together whether the child needs to be seen in person.

Can a nurse practitioner prescribe medication during a virtual visit?

Yes. Our nurse practitioner is licensed to diagnose and prescribe in Colorado. When a prescription is appropriate, it is sent electronically to the pharmacy you choose before the visit ends.

Can you order lab work or an X-ray from a virtual visit?

Yes. Orders for labs and imaging go into your chart during the call, and you can come by the clinic or a nearby lab at your convenience. Results are reviewed by the same provider, and follow-up can happen by video again.

Who will I see on the video call?

The same nurse practitioner who would see you in our Greeley clinic. NOCO Healthcare was founded by Sheree Montoya, FNP-C, and it is a small practice on purpose, so the provider on your screen is the one who knows your chart.

Does NOCO accept Medicaid for virtual visits from Severance?

Yes. We accept Medicaid, Medicare, and most major insurance plans for video visits, billed the same way as an office visit. If you are paying out of pocket, ask for the self-pay rate when you call.

Do I need to download an app for the video visit?

No. The link we text you opens in your phone's browser on a secure, HIPAA-compliant platform. You do not need to create an account or remember a password.

What conditions can be treated by video?

Colds and flu, sinus infections, sore throats, ear pain, pink eye, urinary tract infections, migraines, rashes, allergies, stomach bugs, medication refills, and follow-ups on conditions we already manage. Anything involving chest pain, breathing trouble, possible fractures, or wounds needs to be seen in person.

Are virtual visits only for established patients?

No. New patients are welcome. Call, give us your basic information, and we will schedule your first visit by video. If we need paperwork from you, we send it electronically.

How far is NOCO Healthcare from Severance if I need to come in?

Our clinic is at 1130 38th Avenue, Suite A, in Greeley, about 20 minutes from Severance. If a video visit turns into an in-person visit, we book it before the call ends and hold same-day and next-day appointments for that reason.

Feeling rough in Severance? Do not drive. Call.

A video visit with our nurse practitioner is usually available the same day. If you need to be seen in person, we will tell you and get you in.
