The Sports Physical Form Is Due This Week
Practice starts Monday, the form is crumpled in a backpack, and every place you call is booked out. Your kid is anxious about sitting out, and you are anxious about the whole season slipping away.
Call us and we will look for the soonest weekday opening. Bring the school or league form and any injury history. A family nurse practitioner completes the exam and fills out the paperwork, so your athlete has the form in hand and you can stop chasing appointments.

Kids’ health in Fort Collins: schools, sports, and summer
We see children from across Poudre School District, including families from Fort Collins High School on Lambkin Way, Rocky Mountain High on West Swallow Road, Poudre High on Impala Drive, and Fossil Ridge on Ziegler Road. Great question! We have all of the same offerings as a larger competitor. We can take any labs necessary and provide you with fast results and analysis. Additionally, when you need additional services we do not send you to another department to make another appointment, we handle everything in office.
